2:         Provide via email or hard copy via mail, a headshot photograph to             be used for website and promotional purposes
3:         Complete questionnaire
4:         Complete Recipient Profile
5:         Provide mailing address for Bursar’s office

Procedures to renew scholarships

1:         Provide via email, scanned proof of full time enrollment
2:         Provide letter from qualified non profit 501 C 3 showing no less than            20 hours of community or volunteer service
3:         Provide address of Bursar’s office
